What do we offer?

Advisory sessions for senior care

The world of senior care is complex and emotionally charged, from managing hospital discharge, to choosing care facilities, to interviewing home health care staff, and knowing when to call hospice. We all want the best for our loved ones and it's important to leverage the right resources to advocate for them and to guide informed decisions.
Hospital Discharge
Are you prepared to take your loved one home or choose the right facility for short term rehab? Do you know which facility is rated best for wound care or physical rehab or other needs?
Care Facilities
Which facility will best meet your loved one's needs? Are there choices for admission; skilled versus assisted living? How did those facilities score during their last regulatory survey? What are their staffing ratios?
Home Care
Before you bring your loved one home, consider interviewing multiple agencies for the right fit. What is their CMS quality rating? Are you confident in their service approach? Is the treatment plan comprehensive?
Medication Profile Review
The use of five or more drugs creates polypharmacy and the risk of harmful interactions. Sometimes more medications are prescribed to reduce the negative symptoms caused by too many drugs. A medication review may be helpful to reduce adverse effects and maximize quality of life.
Hospice
Hospice is about living and making the shift in focus from one of cure to one of comfort. Living with a terminal diagnosis includes many decisions about end of life care. Which hospice is right for your family? Must one die at home? What services are coverered by Medicare? Are bereavement services only local?
Transition to Home
What level of mobility and self-care is required to progress at home? How will your loved one safely enter/exit the home? Do you need help finding community resources for transportation, meal prep, housekeeping, or activities of daily living? Remote assessments available.
